ADULTS
When an adult with a learning disability and/or autism wishes to be referred to our services, this can be done either by the person themselves and their family or through a health professional.

A Western Care Association Referral Form will be forwarded by us to the health professional/family in order that contact details, other additional information and formal consent for the referral can be obtained. Following this, the relevant Western Care Association staff will contact the person/family to make an appointment to discuss the person’s priorities. On receipt of a referral, we will then contact the person and their family to arrange an appointment to discuss the person’s requirements.

Homeshare Service
The home share service is where a child or adult spends time, in the home of another family.
It aims to:
- give individuals and families greater control and choice in the support services they receive
- to provide a break for the parent/ carer whilst being a positive experience for the person being supported.
Where the individual is not already registered with Western Care, then the Referrer should follow Western Care external referral process, stating on the referral form that the service required is ‘home share’. Referrals can be made via healthcare professionals from external bodies, including the Children’s Disability team (CDNT) in the case of children. For these external referrals, the most recent psychological assessment stating the individual has a diagnosis of ID and/or Autism is required. Where the child or adult is already registered with Western Care, the Home share referral form should be completed and submitted to the Principal social worker in the first instance.
Further information may be requested before a decision can be made on acceptance for a home share service:
- Western Care Home share referral form detailing specifically the benefits of home share for the individual.
- RIPT (Respite Intake Prioritisation tool) for information regarding individual/family’s needs/prioritisation purposes.
- Completion of ‘Criteria for assessing home share as support option for an individual’ This gives us information regarding potential risk issues and how the home share service can respond to them given that the person being supported will be spending time in a typical family home with a typical family.
